Document on R.E.M:n viides studioalbumi. Se julkaistiin syyskuussa 1987 muutama kuukausi kokoelmalevy Dead Letter Officen jälkeen. Document oli yhtyeen viimeinen studioalbumi I.R.S. Recordsille ja albumi, jonka myötä yhtye nousi lopullisesti merkittävien rockyhtyeiden joukkoon - joulukuussa 1987 R.E.M. oli Rolling Stonen kannessa otsikolla "Amerikan paras Rock and Roll -yhtye".

Document oli ensimmäinen Scott Littin tuottama R.E.M.-albumi. Littin ja yhtyeen yhteistyö jatkui aina New Adventures in Hi-Fi -levylle asti. Document oli ensimmäinen R.E.M. -levy, joka myi platinaa ja levyllä oli yhtyeen ensimmäinen suuri hitti: "The One I Love", joka nousi Yhdysvaltain singlelistan sijalle yhdeksän.
