Middlesbrough FC deltog säsongen 2008/2009 i Premier League, FA-cupen och Engelska ligacupen. Manager var Gareth Southgate, som gjorde sin tredje säsong på tränarbänken. Säsongen innebar nedflyttning från Premier League, på grund av en nittondeplats. Detta innebar att Middlesbrough för första gången på 2000-talet tvingades ta steget ned till Football League Championship. En rad spelare, bland annat egna produkten Stewart Downing och rekordköpet Afonso Alves lämnade klubben efter nedflyttningen, och laget kom att fastna i näst högsta serien i flera år.
